You've summarized a great deal of what has been frustrating me over the past couple of years in terms of UI design. Im glad you didn't mention the horrendous design of modern CRM and ERP systems. When I see an Oracle or IBM (or any company for that matter) CRM/ERP solution, I feel like the designers there have absolutely zero concept of how to design for usability ;) Great post.
